AHRQ Infrastructure for Maintaining Primary Care Transformation (IMPaCT) Grants Catalog of Tools

This Catalog of Tools and Resources brings together the materials developed through the AHRQ Infrastructure for Maintaining Primary Care Transformation (IMPaCT) cooperative grants. These grants supported model initiatives in four States (Pennsylvania, New Mexico, North Carolina, and Oklahoma), which used primary care extension agents to assist with primary care transformation efforts statewide and were then spread to partner States. The materials in this catalog include toolkits for building infrastructure and implementing health extension models, reports outlining lessons learned, Webinars to share information and experiences, descriptions of the various models used, and peer-reviewed articles.

Building Infrastructure to Promote Primary Care Transformation: Lessons From a Four State Learning Community	This report describes the North Carolina model for using extension agents to assist with primary care transformation, and summarizes the value of transformation for the States involved in the North Carolina IMPaCT Learning Community: Idaho, Maryland, Montana, and West Virginia.
