AHRQ's Healthcare Extension Service Logic Model
This Logic Model (PDF, 423 KB) shows how AHRQ's Healthcare Extension Service is intended to provide state-based solutions for healthcare improvement, and is a guide for planning, implementation, and evaluation efforts. The Logic Model identifies the initiatives' planned inputs, such as expertise, collaborative relationships, resources, and infrastructure provided by the initiatives' three components: the Healthcare Extension Cooperatives, National Coordinating Center, and National Evaluation Center. These inputs inform and guide the activities of these components, such as convening stakeholders, technical assistance, learning networks, and project evaluation and monitoring. These activities, in turn, are expected to lead to outcomes such as increasing capacity to provide healthcare extension services, accelerated adoption of patient-centered outcomes research evidence into safety-net healthcare delivery organizations, and reduced healthcare disparities.
