SOPS Databases
AHRQ established the Surveys on Patient Safety Culture® (SOPS®) Databases as central repositories for survey data from each of the SOPS surveys and SOPS supplemental items. Participation in the SOPS Databases is free and open to users of the surveys to voluntarily submit their data to the appropriate database. In addition, organizations that administer any of the supplemental items with the companion SOPS survey can now also submit data to the appropriate database. Participating organizations receive a customized feedback report with their own results compared with the Database results.

SOPS Planned Data Submission Schedule
AHRQ opens voluntary data submission for two SOPS Databases per year. Therefore, each SOPS Database is typically open for submission every 2 years. Organizations that want to participate in the SOPS Databases will have to submit specific data elements and comply with database guidelines. Select the survey name below for detailed information about participating in each of the SOPS databases.

Research Datasets
Researchers may request access to research datasets, which are de-identified data files from the SOPS Databases, to help answer important health services research questions related to patient safety culture, as measured by SOPS surveys. For the SOPS Hospital Database only, hospital-identifiable data files may be made available. All requests for research data must be approved by AHRQ.
