Implementing Primary Care Team Huddles Worksheet

Huddles are brief team meetings to determine what tasks need to get done and by whom. This brief description of huddles encourages and enhances communication among primary care team members, an activity that aligns with Key Driver 4: Create and support high functioning teams to deliver high-quality evidence-based care. It includes a worksheet to help practices identify the goals of huddles, huddle topics and processes, and obstacles to huddling and their solutions.  Practices can also access the Healthy Huddle Warm-up Tool that helps team members gather data in preparation of huddles.
