Ana Quiñones, PhD and David Dorr, MD will be presenting on "Preliminary Findings From Person-Centered Care Planning for People with Multiple Chronic Conditions (PCCP4P)." This webinar will discuss findings from the PCCP4P project, including a high-level overview of models and approaches to providing person-centered care planning (PCCP) for people with multiple chronic conditions (MCCs).
