Jeana Reyes, M.S.N., R.N., C.C.R.N.-C.S.C.-C.M.C., is the director of clinical transformation and quality for clinical reengineering, value-based programs at Horizon Blue Cross Blue Shield of New Jersey. She serves as the key business owner for developing clinical strategies and quality program models that lead to improving quality outcomes for the 1.9 million members attributed to Horizon’s value-based programs. As director, she oversees and leads a cross-functional team that shares responsibility for provider relations, population health, quality data exchange, and healthcare education and data information sets and stars performance within a matrixed organizational environment.
Ms. Reyes, an experienced clinical leader with more than 25 years of healthcare industry experience in acute care, nursing education, population health management, and value-based care delivery, has a passion for adult learning theories, leading to the development of multiple adult learning clinical training programs that foster their eagerness to connect with experience and motivation for self-improvement.
Ms. Reyes has served as the adjunct nursing faculty for an accredited nursing program at a local state college. She is a member of the American Nurses Association, American College of Healthcare Executives, and American Association of Critical Care Nurses, where she served on numerous committees over the past 15 years, including the Standards of Practice Task Force and National Certification Item Writing Committees. She received her master’s degree in nursing with a postgraduate certificate in nursing administration, and a minor in nursing education, from Thomas Edison University and is nationally board-certified in critical care nursing, cardiac surgery nursing, and cardiac medicine nursing.
