This educational module briefly describes eight steps primary care practices can take to form and work with a patient and family advisory council (PFAC). It helps primary care practices increase engagement with patients and families, advancing from consulting patient and family advisors to sharing leadership, in line with Key Driver 5: Engage with patients and families in evidence-based care and quality improvement. This resource contains guidance on how to prepare a PFAC action plan, charter, and budget, as well as how to select improvement projects and track results.
