It helps primary care practices increase engagement with patients and families, advancing from consulting patient and family advisors to sharing leadership, in line with Key Driver 5: Engage with patients and families in evidence-based care and quality improvement. This resource contains guidance on how to prepare a PFAC action plan, charter, and budget, as well as how to select improvement projects and track results.
