Resource: Producing Accurate Clinical Quality Reports for Population Health: A Delivery System-Oriented Approach to Report Validation (PDF, 1.24 MB, 21 pages)
This resource shows how to design internal reports for quality improvement (QI) purposes, as an essential component of Key Driver 2: Implement a data-driven quality improvement process to integrate evidence into practice procedure. It also supports Key Driver 3: Optimize health information systems to extract data and support use of evidence in practice by describing different types of QI reports and different ways of generating them from electronic health records (EHRs). A case study drives home the importance of involving care teams in validating data, and the ability of checklists to help detect both inclusion errors (including patients in the measure that should not have been included) and exclusion errors (patients who should have been in part of the measure but were left out). While aimed at health care delivery systems, this resource can help primary care practices create accurate and valid data reports for internal clinical quality improvement purposes.
