Comprehensive Care Plan Template for Patients and Clinicians

This care planning work sheet helps primary care practices and their patients create plan of action to manage hypertension. By completing the Comprehensive Care Plan together and having patients set their own goals, practices are in alignment with Key Driver 5: Engage with patients and families in evidence-based care and quality improvement. The Comprehensive Care Plan contains an assessment of readiness to make changes, goal setting and tracking, identification of barriers to change, and assesses interest in obtaining additional information useful for self-management.
