This online learning module provides a comprehensive overview and guidance for practices to implement a team-based model of primary care to enhance quality of care and productivity. Resources to support Key Driver 4: Create and support high functioning teams to deliver high-quality evidence-based care—including case studies, toolkits, and videos—are tied to each of the eight action steps for integrating team-based care into primary care practices. For example, the worksheet “Share the Care: Assessment of Team Roles and Task Distribution” helps practices recognize the responsibilities of each team member and redistribute tasks in a way that better fits the practices’ workforce and patients.
