This section of the Web-based Primary Care Team Guide outlines how primary care practice teams can improve the quality of patient care. It contains four action steps to assess the need for and pilot changes using teams. This guide can help practices engage care teams and other staff to support implementation of new evidence, part of Key Driver 2: Implement a data-driven quality improvement process to integrate evidence into practice procedures.
