Project Description: This project will investigate the region-specific determinants of a cost-effective strategy that is critical for the future of emergency care in ST-elevation myocardial infarction (STEMI). There are three main aims:

1. Using a representative sample of US counties, estimate the average and regional effectiveness of alternative emergency medical services (EMS)- and hospital-based strategies for increasing access to percutaneous coronary intervention (PCI), compared to current practice.
2. Within and across counties, estimate the costs and cost-effectiveness of EMS- and hospital-based strategies for increasing access to PCI, compared to current practice.
3. Evaluate relationships between county geographic and health system characteristics and the cost effectiveness of EMS- and hospital-based strategies for increasing access to PCI.

Career Goals: Dr. Concannon is an Assistant Professor at Tufts University Medical School in Boston and an Assistant Professor at the Institute for Clinical Research and Health Policy Studies at Tufts-New England Medical Center. He received a Ph.D. in Health Policy from Harvard University. His immediate career goals are to fill the major knowledge gaps in the emergency management of patients with acute coronary syndromes; to develop planning tolls for regionalization of cardiac care; and to ensure planning tools minimize unnecessary burden on emergency health care resources and are readily adaptable to other clinical domains. The grantee's long-term goals are to identify, develop and implement optimal organizational strategies for a range of high cost and high intensity patients.

Progress to Date: Dr. Concannon has completed three studies related to Aim #1 and a manuscript has been accepted for publication.

Future Plans: The grantee will complete the sampling and modeling of US counties in the near future. Work will continue on the redevelopment of simulation code. Aim #1 will be completed.
