Environmental Scan of Primary Care-Based Efforts To Reduce Readmissions

While emerging literature addresses primary care-based transition programs, a gap remains in what is known in the hospital setting. To address this gap, AHRQ funded an environmental scan on the evidence-base for reducing readmissions in the primary care setting by analyzing the findings of 42 peer-reviewed articles and 30 items from the gray literature.
