The eCare Plan project produced three videos on the following topics:
Video 1: Challenges of Managing Multiple Chronic Conditions
This video introduces Dorothea, who has multiple chronic conditions, and explains some of the difficulties navigating a complex health care system.

This video explains what a care plan is and answers some common questions patients might have about developing and following a care plan.
Video 3: Your Care Plan
This video shows how a care plan can facilitate communication between all members of the care team and touches on the role that health information technology can play in care planning.
