Community Connections: Linking Primary Care Patients to Local Resources for Better Management of Obesity

Tool 11 is intended to combine the best of the practice walk-through and the process map. The rectangles represent each activity in which a casual mention or formal referral of the targeted community partner could be initiated. The circles above the rectangles represent the referral actions that accompany the original activity. The cylinders below the rectangles are intended to serve as a checklist for needed tools, resources, or follow-up steps.

Key:

  • Rectangle = Normal workflow activity in which a casual mention/formal referral could be initiated; could be a part of overall patient flow or within the actual patient encounter. 
  • Circle = Referral actions to accompany the normal activity. 
  • Cylinder = Checklist for needed tools, resources, or follow-up steps.
