Dear Staff, 

As you know, we strive to provide the highest level of care at our practice. Dr. ________ and I have been learning more about the benefits of formally partnering with community resources and concrete ways in which we can do this. I believe it's possible to enhance the care we provide inside the practice by connecting with  community resources outside the practice. The resources I have mainly give advice for helping patients with obesity and diabetes; however, we can discuss the patient population that we think needs this extra help the most.
