Primary Care Landscape: Findings
What does the state of primary care look like for small- and medium-size practices across the 7 regions participating in EvidenceNOW? This section highlights what EvidenceNOW is learning about practice characteristics and care delivery.
Practice Facilitation: Findings
Practice facilitation is a service provided to support primary care practices by a trained individual or team who use a range of approaches and methods to build the practices’ capacity for ongoing quality improvement. Practice facilitation (coaching) was a key strategy used by all 7 EvidenceNOW Cooperatives to help primary care practices improve care delivery Read here about what we’re learning about practice facilitation.
Practice Capacity for Quality Improvement: Findings
As a primary care practice develops understanding and skills about how to use data and feedback to improve, it is developing its ‘QI capacity’. QI capacity also depends on the commitment of practice leadership and staff. External support can help build practice capacity. Access findings about baseline practice capacity and how it changed with EvidenceNOW here.
Improving the ABCS of Heart Health: Findings
EvidenceNOW was designed to assist primary care practices in reaching and exceeding the Million Hearts® target of 70 percent for delivery of each of the ABCS services to improve heart health—Aspirin use, Blood pressure control, Cholesterol management, and Smoking cessation. Check back here for findings related to baseline and improvements in the delivery of ABCS services.
EvidenceNOW evaluators are exploring a wide range of issues that affect primary care practices and the delivery of primary care, from the spread of innovations to burnout in primary care staff. Find out what we’re learning here.
