Videos Tell the Story of EvidenceNOW: Advancing Heart Health
These four videos tell the story of how seven regional Cooperatives worked with more than 5,000 clinicians at over 1,500 small- and medium-sized primary care practices to improve the delivery of heart health services to about 8 million patients. This was accomplished, despite challenges resulting from years of a fragmented health care system, using a model of external support to often resource-strapped primary care practices. The promising results showed improved delivery of the ABCS (aspirin for eligible patients, blood pressure control, cholesterol management, and smoking screening and cessation counseling) for participating practices. Results also showed that this approach improved the capacity of primary care practices to improve quality of care. The blueprint for improving primary care using external support is already being expanded to address other health problems facing primary care practices, such as unhealthy alcohol use.
