The Pediatric Hospital Care Improvement Project (P-HIP) Collaborative collected Hospital-to-Home Transitions data from the eight participating hospitals for nearly 4.5 years in order to analyze results over time. The analysis includes 24 months of data before the interventions were implemented to allow researchers to establish a baseline for comparison purposes. Generally, the quality improvement efforts began in February of 2018.
After implementing the quality improvement efforts, special cause variation was detected with an upward shift (13 points above the center line on the overall statistical process control chart) in the percentage of families receiving discharge instructions with all of the recommended elements included in the quality measure. During the 12 months following completion of the QI collaborative (February 2019 to January 2020), these improvements were sustained.
