Million Hearts®
Million Hearts®, a national initiative co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS) with the goal of preventing 1 million acute cardiovascular events by 2027, is working with cardiac rehabilitation (CR) professionals, public health professionals, quality improvement specialists, patients, and other contributors to achieve a national goal of 70% participation in CR for eligible patients.
The Million Hearts® Cardiac Rehabilitation Collaborative (CRC) is a national forum of multi-disciplinary professionals. The CRC meets virtually on a quarterly basis to increase awareness about the value of CR, spread best practices to increase CR participation, build equity in CR referral and participation, enhance sustainability and innovation, and track progress toward its shared goal.
