AHRQ Safety Program for Long-Term Care: HAIs/CAUTI
The Sustainability Assessment Tool is developed to help you understand what project elements and other factors may influence sustainability. There are two formats to choose from. You may print the handout to write in scores, manually calculate the scores for each domain, and make notes on more specific barriers and opportunities for improvement you identified. Alternatively, you may use the digital version to enter scores that will auto-calculate the average domain score for you with open text fields to identify barriers and opportunities for improvement.
The Sustainability Action Plan template is designed to guide you through the development of an action plan to increase the likelihood of sustaining areas of the AHRQ Safety Program for Long-Term Care: HAIs/CAUTI that you identified as being relevant, important, and feasible to sustain in your long-term care facility.
