Original Training Modules
This page contains links to the ten original TAKEheart training modules that were that developed to support over 100 hospitals as they worked in real time to implement two proven strategies for increasing patient participation in their cardiac rehabilitation (CR) programs.
These ten modules:
- Were designed to accompany the hospital-focused quality improvement approach presented in the Million Hearts®/AACVPR Cardiac Rehabilitation Change Package.
- Contain recordings of the expert-led training sessions (originally held live), the accompanying slides, and topic-specific implementation guides.
- Were presented to hospitals over a ten-month period, with additional AHRQ-provided coaching, as they sought to simultaneously begin to implement or improve their automatic referral and care coordination processes.
To better enable hospitals to focus their efforts on one improvement area at a time, and to proceed independently and at their own pace, the ten modules have been streamlined, reconfigured, and consolidated into four focus areas, with revised slide decks and implementation guides. Hospitals wishing to take a deeper dive into specific topics covered in these consolidated curricula can listen to the original recordings and review the original slides and other materials from which these curricula were developed.
