Following the call to action in Module 1, this module lays the foundation for systems change within hospitals and health systems to successfully implement cardiac rehabilitation (CR) programs. Topics covered in this module include how to:
- Identify a CR champion who takes responsibility for leading the project.
- Form a multidisciplinary CR quality improvement (QI) team, including the ideal composition of the team and why each team member is important.
- Develop an action plan for CR program implementation.
Suggested Audience
The target audiences for this session include:
- The CR QI team
- Cardiologists
- Cardiovascular surgeons and nurses
- Care coordinators
- Discharge planners outside of the team
- Hospital administrators and officials
It is important to note that the audiences listed above are a suggestion rather than a limitation. All are welcome to watch and review the module.
Learning Objectives
Upon completion of this module, attendees should be able to:
- Select a CR champion to lead the CR QI team and advocate for automatic referral with care coordination.
- Identify key members of a multidisciplinary CR QI team, with representatives from key departments and patient advisors.
- Create an Action Plan for implementing automatic referral with care coordination.
