Tools for Building Primary Care
Date
August 22, 2018
Summary
This EvidenceNOW Webinar highlights tools and resources collected as part of AHRQ's EvidenceNOW initiative that can be used by practice facilitators and others interested in helping build primary care practices' capacity to implement the best clinical evidence. The tools and resources are organized using a Key Driver Diagram that allows users to search for resources needed to achieve specific quality improvement goals. Each key driver (for example, create care teams or select and improve evidence) links to specific change strategies and the tools for implementing each of those strategies to improve primary care. The speakers from AHRQ provide background on the EvidenceNOW initiative and development of the key driver diagram and resources. Two practice facilitators share their experiences with some of the collected tools and resources.
