AHRQ Safety Program for Mechanically Ventilated Patients
Date __________ Month __________ Hospital __________ Unit __________
Use this tool to track your progress in reducing ventilator-associated event (VAE) rates by using this information to calculate the incidence rate of VAEs as well as ventilator-associated complication (VAC) events, infection-related ventilator associated complication (IVAC) events, and possible ventilator-associated pneumonia (PVAP) events. Report the following occurrences within the selected month. Collect this data each month for all beds in your unit. The number of VAEs and total IVACs should be reported to the National Healthcare Safety Network (NHSN).
________ VENT DAYS | Total number of ventilator days |
________ VAE | Total number of ventilator-associated events (VAC + Total IVAC) |
________ VAC | Total number of ventilator-associated complications |
________ IVAC (excluding PVAP) |
Total number of infection-related ventilator-associated complications events, excluding PVAP |
________ PVAP | Total number of possible ventilator-associated pneumonia events |
________ Total IVAC (IVAC+PVAP) |
Combined total number of IVAC and PVAP events |
CALCULATING VAE MEASURES PER 1,000 VENTILATOR DAYS | |
---|---|
VAE Rate | [(# of VAC events) + (# of IVAC events) + (# of PVAP events) / (Total vent days)] * 1000 |
VAC Rate | (# of VAC events / Total vent days) * 1000 |
IVAC Rate | (# of IVAC events / Total vent days) * 1000 |
PVAP Rate | (# of PVAP events) / (Total vent days) * 1000 |
Total IVAC Rate | [(# of IVAC events) + (# of PVAP) / (Total vent days)] * 1000 |
Additional instructions for calculating VAE measures can be found in the Data Measures Guide (PDF, 554 KB).
