This infographic provides a visual overview of the program’s three main technical bundles to improve care for mechanically ventilated patients: daily care processes, early mobility, and low tidal volume ventilation. These three technical bundles are structured around the principles of the Comprehensive Unit-based Safety Program (CUSP), an innovative safety and quality intervention that helps teams identify and learn from defects, improve safety culture, and improve teamwork and communication.

AHRQ Safety Program for Mechanically Ventilated Patients. CUSP: Tap the wisdom of frontline staff. Educate staff on the science of safety. Identify defects. Partner with senior executive. Learn from defects. Improve teamwork and communication. Daily Care Processes: SSD-ETT: Use subglottic secretion drainage endotracheal tube if intubated greater than or equal to 72 hours. HOB: Confirm head of bead elevation greater than or equal to 30 degrees. SEDATION: Minimize sedation level. DELIRIUM: Assess then address delirium. SAT: Perform spontaneous awakening trial–Wake up! SBT: Perform spontaneous breathing trial to wean patient off mechanical ventilation. Early Mobility: SEDATION: Minimize sedation level. DELIRIUM: Assess then address delirium. MOBILITY: Tailor goals to maximize mobility. Low Tidal Volume Ventilation: ARDS: Prevent acute respiratory distress syndrome. PEEP: Use positive end-expiratory pressure of greater than or equal to 5 cm H2O. PLATEAU: Maintain plateau pressure at less than or equal to 30 cm H2O. TIDAL: Use tidal volume of 6–8 mL/kg and 4–6 if ARDS present.
