Assessing Progress on CLABSI and CAUTI Prevention
For successful implementation, teams need to assess their progress on CLABSI and CAUTI prevention; you might be just starting this work, or doing a check-in after some time of implementation. Assessing the team’s work helps identify strengths and opportunities for improvement in your intensive care unit’s (ICU) clinical and safety practices. This section has three steps: complete the ICU Assessment, view the ICU Assessment Response Guide to identify resources in this toolkit to support your work, and create an Action Plan based on your assessment results. Since human behavior inherently has inconsistencies and culture is always evolving, regular check-ins using these three tools will help move your team’s central line-associated bloodstream infection (CLABSI) and catheter-associated urinary tract infection (CAUTI) prevention work forward. Periodically doing these three steps will assure you are addressing your current challenges and utilizing your team’s strengths.
ICU Assessment
The ICU assessment helps teams find an area to focus their infection prevention efforts. The unit project team leader should complete it in collaboration with individuals who have strong knowledge of current clinical and safety practices in the ICU. If a unit project team leader has not been identified, unit leadership can work to nominate an individual, or a team member can self-identify as that unit project team leader; consider co-leads to support collaboration and reduction of burden.

ICU Assessment Response Guide
After completing the ICU assessment, use the ICU Assessment Response Guide to understand the rationale behind each question and link resources to specific questions. The assessment used with the ICU Assessment Response Guide will allow teams to identify opportunities that they can address. This will help define the specific areas the team wants to work on to reduce infections.

Action Plan Template
After reviewing the Response Guide, teams can begin to create an Action Plan. This Action Plan will guide the team’s progress toward the reduction of CLABSI and CAUTI. This worksheet will help the team draft a thoughtful statement (the Aim) that will guide the team in its actions and can be adjusted as change occurs in the process to reduce CLABSI and CAUTI.
